2. ​​​​​You will need...

There are standard forms that need to be completed and documents that you need to give us no matter what you’re claiming for. We’ll tell you exactly what we need and you can simply download the relevant forms here: 

• Disability claim form: this needs to be completed by the claimant
• Disability claim: this needs to be completed by the relevant doctor or other medical professional
• Disability claim: this needs to be completed by the claimant's employer
• Policy Owner form: this needs to be completed by the owner of the policy
• A certified copy of the claimant's identity document  
• A bank statement or letter from the bank with a bank stamp confirming the claimant's bank account

Send the documentation listed above to us in one of these two ways:

• You can also submit the documents via your broker or financial adviser

You’ll need to fill in one or more of the forms below, depending on what you’re claiming for. We’ll tell you exactly what we need, and you can simply download the relevant forms here: 

• Business Expense
• Certificate of continued disability
• Certificate of continued disability income
• Certificate of existence

5. ​​​​​What happens next

Send us the forms

Complete and sign the forms then send them to us or your financial advisor along with any of the other documents that we’ve asked for. You can send them to us in two ways:
