How To Claim - Disability Impairment

Making a claim is as easy as 1, 2, 3.
 

1. Get in touch 

Contact your financial advisor or give us a call on 0860 000 011 and let us know what you want to claim for. 

2. Find out which forms to fill in 

There are standard forms that need to be completed and documents that you need to give us no matter what you’re claiming for. 

  • Disability claim form: this needs to be completed by you.
  • Disability claim: this needs to be completed by the doctor or other medical professional that’s treating you.
  • Disability claim: this needs to be completed by your employer.
  •  Policy Owner form: this needs to be completed by the owner of the policy.
  • A certified copy of your identity document.
  • A bank statement or letter from the bank with a bank stamp confirming your bank account.
     

You’ll need to fill in one or more of the forms below, depending on what you’re claiming for. We’ll tell you exactly what we need and you can simply download the relevant forms here: 

3. Send us the forms 

Complete and sign the forms then send them to us or your financial advisor along with any of the other documents that we’ve asked for. You can send them to us in two ways:
