How To Claim - Critical Illness

1. Get in touch 

Contact your financial advisor or give us a call on 0860 00 00 11 and let us know what you want to claim for.   

2. Find out which forms to fill in 

• Critical Illness claimant form: this needs to be completed by you
• Critical Illness medical attendant claim form: this needs to be completed by the doctor or other medical professional that’s treating you
• Policy owner form: this needs to be completed by the owner of the policy
• A certified copy of your identity document
• A bank statement or letter from the bank with a bank stamp confirming your bank account

 3. Send us the forms 

Complete and sign the forms then send them to us or your financial advisor along with any of the other documents that we’ve asked for. You can send them to us in two ways:
