There are standard forms that need to be completed and documents that you need to give us no matter what you’re claiming for. We’ll tell you exactly what we need and you can simply download the relevant forms here:

• Critical Illness Claimant Form: this needs to be completed by the claimant.
• Critical Illness Medical Attendant Form: this needs to be completed by the relevant doctor or other medical professional.
• Policy Owner Form: this needs to be completed by the owner of the policy.
• A certified copy of the claimant's identity document.
• A bank statement or letter from the bank with a bank stamp confirming the claimant's bank account.
