Step 2 - Send us the required documents

To start the claims process we will need the below standard documents from you. We may require additional documentation depending on the nature of the claim, but these will be requested by the assessor.

Documentation needed for a life cover claim:

• Death clai m form, completed by the
•  Death claim form, completed by a medical attendant 
• A certified copy of the death certificate
• A certified copy of the identity document of the insured and claimant
• A letter of executorship (if no beneficiaries)
• Proof of bank account of the claimant (bank statement or letter from the bank with a bank stamp)
   

Documentation needed for a funeral claim:

• Funeral claim form, completed by the claimant 
• A certified copy of the death certificate
• A certified copy of the identity document of the insured and claimant
• Proof of bank account of the claimant (bank statement or letter from the bank with a bank stamp)
   

Additional documents in case of unnatural death:

• Police report (accidental death)
