Please notify Hollard within 24 hours of the insured person’s death by contacting us in one of the following ways:

To start the claims process, we will need the following standard documents from you. We may require additional documentation depending on the nature of the claim, but this will be requested by the assessor.

For a life cover claim:

• Death Claim Form: This needs to be completed by the claimant.
• Death Claim Form: This needs to be completed by the relevant doctor or other medical professional.
• A certified copy of the death certificate.
• A certified copy of the claimant's identity document.
• A certified copy of the insured’s identity document.
• A copy of the DHA-1663 (notice of death) form.
• A bank statement or letter from the bank with a bank stamp confirming the claimant's bank account.
• If there are no beneficiaries, then the claimant will need to provide a letter of executorship.
   

For a funeral cover claim:

• Funeral Claim Form: This needs to be completed by the claimant.
• A certified copy of the death certificate.
• A certified copy of the insured's identity document.
• A certified copy of the claimant's identity document.
• A copy of the DHA-1663 (notice of death) form.
• A bank statement or letter from the bank with a bank stamp confirming the claimant's bank account.
   

Additional documents in case of unnatural death:

• In the case of accidental death, the claimant may also have to complete a Police Report Form.
