There are standard forms that need to be completed and documents that you need to give us, no matter what you’re claiming for. We’ll tell you exactly what we need and you can simply download the relevant forms here:

For a life cover claim:

• Death Claim Form by Claimant: This needs to be completed by the claimant.
• Death Claim Form by Medical Attendant: This needs to be completed by the relevant doctor or other medical professional.
• In the case of accidental death, the claimant may also have to complete a Police Report Form.
• A certified copy of the death certificate.
• A certified copy of the claimant's identity document.
• A certified copy of the life insured’s identity document.
• A copy of the DHA-1663 (notice of death) form.
• A bank statement or letter from the bank with a bank stamp confirming the claimant's bank account.
• If there are no beneficiaries, then the claimant will need to provide a letter of executorship.
   

For a funeral cover claim:

• Funeral Claim Form: This needs to be completed by the claimant.
• In the case of accidental death, the claimant may also have to complete a Police Report Form.
• A certified copy of the death certificate.
• A certified copy of the claimant's identity document.
• A certified copy of the life insured’s identity document.
• A copy of the DHA-1663 (notice of death) form.
• A bank statement or letter from the bank with a bank stamp confirming the claimant's bank account.
