Making a claim is as easy as 1 2 3 

1. Get in touch

Contact your financial advisor or give us a call on 011 351 5000 and let us know what you want to claim for.

2. Find out what forms to fill in

There are standard forms that need to be completed and documents that you need to give us no matter what you’re claiming for. We’ll tell you exactly what we need and you can simply download the relevant forms here:

For a life cover claim: 

  • Death claim form: this needs to be completed by you
  • Death claim form: this needs to be completed by the relevant doctor or other medical professional
  • A certified copy of the death certificate
  • A certified copy of your identity document
  • A certified copy of the life insured’s identity document
  • A bank statement or letter from the bank with a bank stamp confirming your bank account
  • If there are no beneficiaries, then you will need to provide a letter of executorship 
  • In the case of accidental death, you may also have to complete a police report form

For a funeral cover claim:

  • Funeral claim form: this needs to be completed by you
  • A certified copy of the death certificate
  • A certified copy of your identity document
  • A certified copy of the life insured’s identity document
  • A bank statement or letter from the bank with a bank stamp confirming your bank account
  • In the case of accidental death, you may also have to complete a police report form

3. Send us the forms

Complete and sign the forms then send them to us or your financial advisor along with any of the other documents that we’ve asked for. You can send them to us in two ways:
