Life Insurance - How to claim
Times like these can be very stressful so, we’ve taken out the hassle, to make it as worry-free as possible to claim.
You can call us on 0800 252 467 to submit your claim or for any assistance or questions who might have on your claim.
We can only start working on your claim after we have received all the important documents we required. You will need to provide us with thefollowing documents as soon as possible so that we can finalise your claim as quickly as possible.
For a life cover claim
- A Death Claim Form, completed by the beneficiary, policy owner or executor
- A Certificate of Medical Attendant, completed by a regular doctor or physician
- A copy of the Identity Document of the insured and the beneficiary
- A copy of the Death Certificate
- The original policy document
- A letter of Executorship, if this is applicable
- A letter confirming the solvency of the estate if the benefit exceeds R50,000
- An Electronic Funds Transfer (EFT) Authorisation, there’s a link to an EFT form here
- And finally, a Certificate showing the outstanding loan balance, if this is applicable
In the case of unnatural death there are additional pieces of paperwork you’ll need:
- A Police report
- A valid driver’s license where applicable
- A Road Traffic Accident Report if the cause was due to a motor accident - Police Declaration Form
- And a full Post Mortem Report
Making a disability claim
- A Disability Claim Form from the employee
- A Disability Claim Form from the employer
- A Disability Claim Form from the Medical Attendant, which must be completed by your GP
- A Certified copy of your Identity Document.
- All supporting medical reports
- A Boarding Certificate
- An Electronic Funds Transfer (EFT) Authorisation
Making a serious illness claim
- A Dread Disease Claim Form
- A Certificate of Medical Attendant, completed by a regular doctor or physician
- Certified proof of age
- An Electronic Funds Transfer (EFT) Authorisation
All supporting medical evidence dependant on condition
- Heart Attack (ECG tracings, raised enzyme test results)
- Stroke (all specialist reports)
- Cancer (histology reports)
- Renal Failure (all specialist reports; all pathology reports)
- Paraplegia (all specialist reports)
- Blindness (all specialist reports & investigations)
- Coronary artery disease requiring surgery (all specialist reports)
- Surgery for a disease of the aorta (full operation report)
- Replacement of a heart valve (full operation report)
- An organ transplant (full operation report; all specialist reports)
- Terminal Illness (all specialist reports)
- Multiple sclerosis (all specialist reports)
