How to claim on your policy

To make an insurance claim on your DAY-AID Hospital Cash Back Plan policy, follow these simple steps:

• Download – Select the Download claims form links below to download the required claims documents
• Fill in the required forms – You or your beneficiary are required to fill in the relevant forms for your claim
• Send back – Send these completed forms together with necessary documents specified on the claims form in order for us to assess the claim

• Let us get back to you – Please note that claims are payable at the end of the period of hospitalisation or when 30 (thirty) days benefit is due, whichever is earlier
