We'll let you know what paperwork is needed. Forms can be downloaded below.
- Disability claim form from the employee
- Disability claim form from your employer
- Disability claim form from the medical practitioner, which must be completed by your GP doctor
- A certified copy of your Identity Document (ID)
- All supporting medical reports
- A boarding certificate
- Police report
- A valid driver's licence where applicable
- A road traffic accident report to be completed by the investigating police officer, if the injury was caused by a motor accident
