How to claim accident and disability insurance

At Hollard we’ve made it straightforward and as easy as possible to make an accident and disability claim. An accident can be very stressful, so we’ve taken out the hassle to make disability insurance claims as worry-free as possible.

Making an accident and disability insurance claim

You will need a few pieces of paperwork to make your accident and injury insurance claim as smooth as possible. You can download many of the necessary form below:

• A Disability Claim Form from the employee
• A Disability Claim Form from the employer
• A Disability Claim Form from the Medical Attendant, which must be completed by your GP doctor
• A Certified copy of your Identity Document.
• All supporting medical reports
• A Boarding Certificate
• An Electronic Funds Transfer (EFT) Authorisation
• A Police Report.
• A valid driver's licence where applicable
• A Road Traffic Accident Report to be completed by the investigating police officer, if the injury was caused by a motor accident
